Effective September 1, 2020. Care Navigators is an established care management company,
with a separate firm Care Guardians, NFP for guardianship and Power of Attorney services.
Our mission is to provide comprehensive healthcare and advocacy for our clients. This allows
us to expand our services to DuPage and Kane County. We will be able to provide nurses for medical support. Clients will benefit from expanded services including NP/MD visits and
counseling both covered under insurance.
