They maximize client strengths and work together in skill building and problem solving. Luise Warren, MS, LCPC, CMC, RG has 21 years of experience in caring for seniors. She has provided care management within the following settings: home, retirement community, assisted living, partial hospital, and geropsychiatric inpatient unit. She specializes in care management, grief-loss counseling, transition adjustment and cognitive-behavioral therapy. She has led caregiver support and grief groups. She is a past president of the Association of Senior Service Providers, DuPage, and the Kane County Senior Service Providers. National Guardianship Association, Illinois Guardianship Association. Member of the American Society on Aging. Joan Jackson, MSW, LCSW, CMC, RG, C-ASWCM has built an in-depth knowledge of care management by providing social services in such diverse settings a hospitals, nursing homes and community agencies. She trained as a hospice volunteer and is currently a volunteer on a crisis line. She is an adjunct professor in gerontology at the College of DuPage and sits on the Advisory Board for the long-term health care program. She is a member of the National Association of Social Workers and National Guardianship Association, Illinois Guardianship Association. Sarah Baldwin, MSW, LCSW, CMC has 20 years of experience working with the elderly and their families She has provided social work services at senior centers, home health agencies and hospitals.
